Dear fellow practioners:
As you know behavioral health services and mental health services for persons who are hard of hearing, deaf or deafblind are scarce. In this AHRQ funded study, the authors are trying to get a picture of the provision of services by agencies that serve people who are hard of hearing, deaf or deafbind. They want to learn what services exist curently and how service development has occurred around the country.
If you know of any behavioral health providers in your area who provide services to this community, please forward this link to them:

After completing the survey, the respondent will receive a small token of appreciation for their time. We will share the study results upon completion of the study.
Thank you very much for completing the survey or helping us to identify appropriate providers
